Bereavement Ministry The Bereavement Ministry assists mourning families in requested areas of need during times of death within a family. Duties include assisting in the preparation and completion of funeral arrangements, responding to specific needs of the family, participating in the eulogistic services when requested, and providing ongoing spiritual and emotional support. At the bottom of this page is a form for the purpose of sharing information with the Mt. Level church family for both immediate family members (spouses, children, parents, grandparents and siblings) and extended family members (aunts, uncles, nieces, nephews, cousins). The person completeing the Bereavement Ministry Form should be at least 21 years of age, and a designated member of the family responsible for submitting the form.
